We value and wish to honor your time! This is why we offer online patient forms that you can complete at your convenience prior to your appointment. You’ll spend less time in the waiting area, streamlining your initial visit. These forms help us to better understand your background allowing us to provide you with the most effective care possible. When you schedule your initial visit you will receive an email with a unique link to fill the paperwork out online.
If for any reason you are unable to fill out the forms via that link, you can print the forms off from our website to bring with you to your visit. You can also email completed forms in advance to office@WilliamsBCS.com. As always, please do not hesitate to contact us is you have any questions about our patient forms or your upcoming appointment.
Payment is due at the time services are rendered.
If dental insurance will be utilized the patient’s estimated out of pocket portion is due at the time of service.

Williams Family Dentistry works with most insurance companies and as a courtesy to our patients will file the insurance claim on your behalf.
Dental insurance is designed to cover a portion, but not all, of your dental expenses. Most dental insurance policies include a co-payment and deductible, which the patient will be responsible for.
Our office will estimate your primary insurance benefits for each visit based off of the information we receive when verifying your dental benefits. The patient will be expected to pay their estimated portion at the time of service. The patient is ultimately responsible for any amount unpaid by their insurance plan.
If you are unsure of how your dental policy works please ask. Our staff would be happy to answer any of your questions.
